Milk Order

2011 - 2012 MILK ORDER FORM

Milk will be available to the students at the rate of $25.00 for the entire school year per child (there is no milk served on 12:30 dismissal days). If you wish your children to receive milk at school this year, please indicate below the child's name, grade and the choice of white or chocolate. Please return this form with a check made out to St. Mary's. Milk will be available to the children beginning August 30, 2011. Occasionally due to circumstances beyond our control, there may be occasions when milk is unavailable. Notice is given in advance if possible.
